The Live Well Senior Program at Adventist Health Glendale is a wellness membership program that helps older adults stay healthy, independent and socially connected. Founded in 2011 as an educational program for seniors, Live Well serves adults ages 60 and older who live in Los Angeles County. Most members reside in Northeast Los Angeles and Glendale, the program’s primary service area. Today, the program has over 2,300 active members, welcomes 200+ new members each year and offers 25 classes per month focused on fitness, nutrition demonstrations and educational seminars on preventive care. With an emphasis on early identification of health needs and support in navigating care, participants also benefit from hospital-connected services such as health screenings and assistance in finding providers. Through group classes, community events and social activities, Live Well keeps seniors active, informed and engaged in their community.
Why We Did It
Seniors are among the most vulnerable members of communities worldwide, according to the U.S. Census Bureau. As people age, they face greater risks of health problems and disease. The Program was developed to shift from a reactive to a proactive approach to supporting older adults’ well-being. It helps them navigate a complex healthcare system, manage ongoing health conditions, keep up with routine screenings and stay socially connected. It also addresses memory and cognitive changes, mobility and fall prevention, chronic disease management, and guidance on insurance, specialty care and future planning.
