CommunityConnect is a program that helps Medi-Cal patients by placing Lead Care Managers (LCMs) in communities and hospitals. These managers work directly with patients to assess their needs and connect them with essential resources like housing, food, transportation, and legal aid. The program focuses on helping people who have trouble getting care because of challenges like mental illness, homelessness, or frequent hospital visits. LCMs build personal relationships with patients, create individualized care plans, and support them throughout their health journey. In 2024, the program will grow to serve more counties and increase the number of patients it supports. Currently, CommunityConnect helps nearly 350 patients, which will rise as the program expands.

Why we did it:

Adventist Health started CommunityConnect to help Medi-Cal patients with challenges like finding housing, food, transportation, and legal help. Many people in these communities face difficulties in getting the care they need due to complex issues like mental health challenges and unstable living conditions. By connecting them with care managers who work directly in their communities, we can provide more personalized support and help them overcome these barriers. The goal is to improve their health and well-being by giving them the necessary resources to live healthier lives.
