At Adventist Health Clear Lake’s Hope Center, the mission is in the name. Hope Center is the first low-barrier transitional housing program in the county, offering safety and security to people experiencing homelessness. Since its opening, the 24/7, 21-bed center has served 352 individuals, with 121 successfully transitioning into permanent housing. The center provides health screenings, meals, and access to case managers and housing navigators who assist with overcoming barriers to housing, education and employment. The Hope Center received funding through the Providing Access and Transforming Health (PATH) program and the Capacity and Infrastructure Transition, Expansion, and Development (CITED) initiative. This support has helped expand services by assisting participants with housing deposits, health navigation services and continued support after medical care. With $6.3 million in PATH CITED 2 funding, the center is now expanding to increase capacity to 50 beds, allowing it to serve even more individuals in need. After leaving the Hope Center, people are paired with a housing sustainability specialist who provides support for up to six months.

Why We Did It

In Lake County, over 1,000 people qualify as unhoused, 500 of whom are children. This population is often living paycheck to paycheck and faces hardship and danger while living on the street. As a solution, The Hope Center serves as a place where people can walk in and connect with housing resources and support, which is often the first step to finding stable housing. Supporting residents through the entire housing process, the center helps residents build skills through budgeting guidance and education planning. The center also offers services in employment support and behavioral health. These services work together to give residents the support, skills and stability needed to transition into permanent housing.
